I'm playing the new beta version angband at the moment (more of which anon), but I started a new character, and on its first trip into the dungeon...
Blue Dragon Scale Mail (-2) [30,+10] (charging)              
(lying on the floor at 200 feet (level 4))                                     
                                                                               
Provides resistance to lightning.                                              
Cannot be harmed by acid, electricity, fire, cold.                             
                                                                               
When activated, it allows you to breathe lightning for 100 damage.             
It takes d450+450 turns to recharge after use.

ETA: I have to record the following, on descending to the next level (and finding myself in a room full of novice priests, which would normally be bad news):

This seems a quiet, peaceful place.

BDSM on level 4 does suggest that something isn't working quite right, yes. OTOH, if that's the only thing that turns up at totally the wrong depth, it should make the next few levels much easier :)

Remember: it has always been theoretically possible for the One Ring to appear on D:1 (though it's unlikely enough that whether it's actually possible or not depends on the properties of the PRNG being used). BlueDSM on D:4 is remarkably unlikely, but within the realms of "cooooool" rather than "OMG killer bug".

I could have sworn that Angband was set up so that each item/creature had a property that assigned the shallowest depth at which it could occur (to stop you getting nuked by a blue D on level one:), but it must be at least a decade since I last looked at the source, so I might well be thinking of a different roguelike. (Or one of the many *band variants, since I did mostly play Angband and derivatives.)

Yes, but there are various provisions for monsters and items alike to appear OOD with low probability. IIRC, there is a hard cap on OOD monster generation, but none on item generation (although as Martin observes, the nominal probabilities are so low that the actual probability might be zero with a poor RNG, and last time I saw a coherent cut through the tangle of Angband's item generation, some items (like the One Ring) are so rare you might play your whole life without seeing them - hence the chance of hitting one of them on DL 1 is so trivial it is effectively impossible).
